Preparing for Your Consultation & Surgery
We look forward to seeing you at Clearpoint Health Network. In order to help streamline the consultation process, we ask that you gather your medical paperwork in advance of your consultation. We require copies of your bloodwork, diagnostics, and medical records before scheduling a consultation with one of our surgeons.
Before Your Consultation
After you request a consultation for surgery at one of Clearpoint Health Network’s surgery centres, our team of patient care coordinators will reach out to you for more information and to understand your needs. Before they can schedule your consultation with a surgeon, we require you to provide copies of your medical records. Our team will send you a list of the necessary paperwork via e-mail.
Steps to Prep for Your Consultation & Surgery:
- Obtain the required medical documentation listed in our email correspondence to you.
- Forward your medical documentation to your patient coordinator. She will then schedule your consultation with the surgeon.
- During your consultation, you can ask the surgeon any medical questions you may have regarding your procedure and determine if you would like to proceed with surgery.
- The coordinator will contact you to schedule you in for surgery and address any last-minute questions followed by a surgical confirmation email.
